Care coordination can include meetings with members of an individual’s support team (therapist, school, physician, extended family, etc.) as well as meeting notes and debriefs about what was decided.

This highly flexible service allows families to take a back seat in the logistics of a situation which in turn allows them to be fully present for their child or loved one. This service may also include assistance with applying for state and federally funded services which can help families find sustainable and insurance covered services.
